The Preparing for Adulthood and Transition Project is funded by Ealing Council.  We provide, lead on and support information, advice and guidance for young people, parents and professionals regarding transition and preparing for adulthood.

This means working with young people and their families and lots of different groups to make sure everyone knows what Preparing for Adulthood is, where to get support and making sure young people with special educational needs and/ or disabilities understand their rights and options as they become adults.

We do this by:

  • Making sure information is accessible,

  • Using lots of different ways to get information out there like Social Media

  • Meeting with young people and families and helping them to understand their rights and where they can find information

  • Working with professionals so they understand the best way to support young people as they move into adulthood.

  • Working with schools, the local authority and all other groups to make sure young people and families have the opportunity to and are able to access support as they become adults.

Working In Partnership

We work in partnership with the Local Authority and within the community, supporting the strategic direction of transition and preparing for adulthood, policies and procedures.  This means working with the Local Authority and other groups to make sure the right information and support is available to all in Ealing.

We do this by:

  • Listening to and working with young people and families in Ealing to understand what is needed and what is missing in Ealing

  • Knowing what the law and best practise says regarding supporting young people with preparing for adulthood

  • Attending meetings with the Local Authority and contributing using our specialist knowledge

  • Working together so we can all better support young people in Ealing as they become adults
